Important Update for Blue Cross Blue Shield HMO Patients

Effective January 1, 2025, Blue Cross Blue Shield (BCBS) HMO sites currently managed under NorthShore Physician Associates (Site 447) and Illinois Health Partners (Site 243) will transition to a single system-wide site named Endeavor Health Physician Partners (Site 447).

What This Means for You:

• There are no changes to your provider network or benefits.
• You will continue to see your current Primary Care Provider (PCP) and access the same network of care.
• BCBS will send updated insurance ID cards to all affected patients reflecting the new HMO site name.

This update requires no action on your part, and your care with Innovative Counseling Partners will continue without interruption.
